What to expect
Forms & Arrival Time
New patients will be asked to complete New Patient Intake & Consent forms. To save you time, we recommend downloading it HERE and bringing your completed forms with you for your first appointment. If a printer is not accessible to you, we will also have copies upon your arrival.
If you will be filling out your forms in the clinic, please arrive 15 minutes prior to your scheduled appointment. We ask established patients to arrive at least 5 minutes ahead of time to allow for check-in time.
What to wear
You will be treated in the privacy of your own treatment room. Feel free to wear whatever makes you feel comfortable — loose fitting clothing with easy access to your knees and elbows is a good idea.
If you cannot make your appointment we ask that you cancel with at least 24 hours notice. Less than 24 hours cancellation notice or missed appointments will be billed at full rate. If you know that you will be late, please call. We will make every effort to reschedule you for a later time.
